Use मेरा नाम … है to say 'My name is …'. Here मेरा means 'my', नाम means 'name', and है is the verb 'is'. The verb comes at the end: मेरा नाम एना है।
Asking politely and saying where you are from
To ask someone's name politely, say आपका नाम क्या है? — आपका = 'your' (polite), नाम = 'name', क्या = 'what', है = 'is'. The question word क्या stays before the final है. To say where you are from, use मैं … से हूँ. For example, मैं भारत से हूँ। = 'I am from India.' The word से means 'from' and comes before हूँ. To ask 'Where are you from?' politely, say आप कहाँ से हैं? — कहाँ = 'where', से = 'from', हैं = polite 'are'.

Common mistake
Putting the verb at the beginning, as in English: हूँ मैं एना।
